Julie,
Digitizing from a photo presents several problems:
1) the photo may have been taken from an angle that distorts the subject
2) there are rarely solid areas of color; more often it's thousands of shades of various colors
3) in cases like cars & trucks, shadow maybe hiding a lot of detail
4) there are fewer 'hard edges' to define shapes

How much it will cost depends on the usual factors; as to who to recommend, I'm not sure...

This company uses the "photo stitch technique" which looks pretty cool, though I've never purchased a design from them to know how well they stitch. http://www.advanced-embroidery-designs.com/index.html

As for price, it'll be a lot due to the fact that it's from a photograph. It'll also depend on how large you want it, and how detailed.

Be wary of those 'automagic' fixes; results varying from awesome to nightmare-of-thread-breaks
Also, the site you have linked says that BW is best for this technique and 'up to 3 colors' is possible.
Better off going with a digitizer you know/trust! I've done a bunch of designs from photos and it's rarely easy/straightforward.

They are definitely a beast. We embroidered an image of a baby's face done using the photo stitch technique that we bought from some site (I can't recall which one) and it was incredibly dense and made the fabric all wavy. It's definitely not something you want to put on a light weight garment.
